IV Cannulation in Paediatrics Explained

Your team must contend with patient fear, parental distress and tiny veins.
Here are some general tips to make paediatric cannulation safer and easier: π
1οΈβ£ Roll with the punches π₯
This can be a stressful task for the patient, the parent and your team. There is a higher failure rate than adult cannulation. Remember to stay positive and reassure the child and parents where necessary. If it takes multiple attempts, take a deep breath and keep smiling.
2οΈβ£ Ensure the local anaesthetic cream has time to work! β°
EMLA or AnGel cream needs 45 minutes to numb the skin effectively, which means it’s important to communicate with the paediatric ward about timing! Consider changing the order of elective cases if the cream hasn’t had time to work!
3οΈβ£ Understand the child’s needs! π§
Different age groups require different approaches for distractions or explanations. See the article in our bio from the RCH about reducing paediatric anxiety during procedures. It is possible to overload the child, so keep them focussed, keep the mood light and reassure the parent. In most cases, there is no reason for the child to see the IV equipment.
4οΈβ£ Be prepared π«‘
The anaesthetist will be focussed on patient and vein and may not have a spare cannula. Take note of the type and size cannula the anaesthetist is using and take a couple of spares to the bedside. 5οΈβ£ Secure the cannula π
You don’t want to go to all this trouble cannulating to then have it pulled out by a moving limb! Use an appropriate dressing with a flushed extension line and protect it with a bandage in the postoperative period. A splint may be helpful for infants.
